A number of actions have been identified to reduce the 28 day rates of readmission and these include
- Further development of locality teams;
- Continue to develop moving assessment to community; and,
- Assessment / review process to ensure appropriate package of support
Additionally, analysis and national benchmarking has been undertaken to inform the development and targeting of any further improvement actions.
